mbi Medical are pleased to be working with Neath Port Talbot Council in sourcing a level 2 Social Worker to join their team.
The Role
The Social Services and Housing Directorate provides a wide range of services to adult in need and their carers. As a social worker/senior social worker within the Directorate you will play a key role in assessing the needs of individuals, arranging suitable support packages based on identified outcomes of the individual, identify appropriate resources to meet those outcomes and keeping under regular and systematic review the extent to which outcomes are being delivered.
Develop and maintain joint working arrangements with other professionals and agencies.Identify and understand community resources in the locality of the individual to support the delivery of the outcomes identified in the support plan.Attend meetings, including case conferences and make appropriate professional contributions to the proceedings.
Undertake assessments that identify outcomes in partnership with the individual, family, and carer and other professionalsUndertake financial assessments within the charging frameworks of the Council
Identify and jointly agree desired outcomes on the basis of assessed need.
Ensure that all social networks are identified so that the outcomes are deliveredEnsure that all relevant community resources are identified in order that the outcomes are delivered.
Identify and co-ordinate the assistance and interventions necessary meet needs and desired outcomes.
Specify responsibilities for delivering the care plan to meet needs and desired outcomes. Carry out social work casework appropriate to assessed need. Arrange the provision of services to meet need within the resources available and within the framework of the policies and procedures of the service.Initiate appropriate action to protect service users at risk.Manage a caseload efficiently in line with local expectations, e.g. response framework, Positive Risk Taking FrameworkMaintain an electronic calendar so that colleagues and supervisors know, at all times, your location and activity undertaken. Make best use of your time and resources to deliver proportionate and efficient services.
Monitor and review care plans as appropriate in accordance with Directorate policies and guidance.
Maintain accurate and up to date records that provide evidence of need and risk and give a full explanation of the decisions taken.
Complete required documentation within relevant timescales and comply with care management standards.
Manage time and prioritise work demands.
Contribute to the development of the team and people’s learning, including taking responsibility for personal continuous professional development.
Identify shortfalls in available services and unmet need.
Contribute to the monitoring and improvement of services.
Be aware of and actively promote the Authority’s equality policies and reflect equality principles in one’s own attitude and behaviour.
